DQ1 Open and clear communication is critical for the effective functioning of the interprofessional team and the delivery of safe patient care. Discuss the way communication technologies can enhance coordination of care by interprofessional teams. Be sure to discuss a specific in your response.

At the current hospice home care agency I work at we implemented the use of a HIPAA compliant messaging system called Qliq about a year ago. This system can be accessed on any laptop or mobile phone and allows all staff in our office, both clinical and non-clinical, to communicate about patients instantly. Prior to the use of this technology, the only way we could communicate about patient information was either in person, over the phone, or through a specialized type of email within the electronic health record. With most of our patients in their own homes and our nurses constantly in the field unable to always be logged into our electronic health record, nurses were often waiting for long periods of time on the phone trying to speak with other members of the team working out of our office. Once they were able to access Qliq from their phones, they could easily message back and forth with managers and physicians in our office about simple patient questions or issues. This technology also allows our nurses to send a quick message for non urgent issues in a matter or seconds, instead of having to wait until they are able to access the electronic health record again. This technology also allows our nurses to spend more time caring for patients and families since they spend less time being on the phone with our office. More time spent with patients and families ultimately leads to better patient outcomes and higher satisfaction from our patients and families.

When I was working on a medical-surgical unit in the hospital we found that our hospital wide rates of central line infections were increasing. This is tracked as part of the reportable data that CMS requires of all hospitals, and now impacts reimbursement to the hospital. The desire to decrease these rates was important both from a reimbursement perspective and also from a patient outcome perspective. After reviewing the most recent research, the hospital opted to create a central line team that became solely responsible for all central line dressing changes. The staff nurses were also taught to start using alcohol swab caps on all central line ports and ends of IV infusion lines. The combination of these two interventions were successful in decreasing the central line infection rate for the hospital. The staff nurses appreciated that the interventions implemented were not solely giving the nurses more tasks, and instead looked at the hospital as a whole and developed a plan that incorporated multiple different staff in working towards a common goal.

In our facility quality measurement is what helps us make sure that our patients are maintaining their health. We have weekly weights as a measurement that indicates a person’s nutritional status and ensures adequate food and water intake. If a patient is noted to have lost weight, we look in their EHR so see what their diagnosis is. Sometimes a person who has heart failure will fluctuate up or down in their weight related to fluid overload. If a person’s weekly weight measurement is decreasing we look for documentation about how much of their daily meals they consume, also found in the EHR under patient’s daily charting by the nurses and CNAs. We have had good patient outcomes because our facility monitors this very closely.
