The Shift to
There are over 4 million registered nurses in the US, making nursing the leading profession considering the number of health care professionals. Registered nurses work and have leadership responsibilities in all areas of care; from school health rooms to home care, community clinics, and acute care hospitals such as intensive care units. In many rural and remote settings, advanced practice registered nurses are always the only providers in underserved areas alike (Denisco & Barker, 2016). Thus, any shift concerning the working of health care system has a far-ranging consequence on how RNs learn, train, and practice. Given the many roles and its size, the nursing profession has a room for a leadership position in a value-based shift. According to Porter-O Grady and Malloch (2018), the term value-based care has been in discussions regarding ways to improve care with no clearly accepted definitions or measures for what it actually means. There has been less progress to adopting value-based care since many patients, providers, hospitals, employers, insurers, and policy makers have no common ground on the definition of value or the elements that indicate value.
Moreover, different stakeholders have different definitions for value, which might affect future practice. They base their definitions on cost, accessibility, quality, patient satisfaction, and outcome. Therefore, advanced registered nurses are better positioned towards the shift since they are not limited to writing prescriptions and conducting tests, rather, they are situated close to patients and have time to communicate with patients about health, results, and available care options. APRNs also make use of expanded hours and telehealth to coordinate treatments and avoid errors or confusions while developing effective, quality, and safer care plans (Denisco & Barker, 2016). They have developed groundwork for expansion towards patient-centered care and innovation in a system that is resource effective and outcome focused. Hence, they continue to influence the direction of care through a system that leverages information technology to support patients and practitioners in building collaborative network across care settings.

In the age of rapid technological advancement, we are all working in healthcare during a pivotal period of systemic development. Technology and healthcare are colliding and becoming increasingly more intertwined, especially with the ever-increasing use of informatics and EHRs. One quality standard that is of utmost importance to use in the NICU and pediatrics world is that of immunization tracking. In fact, immunization tracking is a Core Quality Measure developed in collaboration with the National Quality Forum, America’s Health Insurance Plans, and the Centers for Medicare and Medicaid Services (CMS) (Core Qualities Measurement Collective, 2020) as reported on the AHIMA website.
The dawn of EHRs and informatics has made it much easier to track a child’s immunization status and allows kids to get their immunizations from different organizations throughout their lives. Gone are the days of paper immunization consent sheets or tables filled out with each specific vaccination’s detailed information. There is a recommended immunization schedule beginning at birth, and though we do not have toddlers and children in the NICU, infants are on a specific schedule that can be challenging to coordinate between team members and providers. EMRs and the use of informatics for tracking purposes provide a centralized place to document immunizations that communicate with the state immunization information system. This aligns with the goal of the CMS Meaningful Use—now called Data Interoperability—is to increase communication between providers and government public health agencies (Centers for Disease Control and Prevention, 2021). Electronic reporting of public health is being improved by the use of informatics and EHR tracking of pediatric immunizations.

I do agree with your elaborate discussion on how use of information technology especially the incorporation of electronic medical records (EMRs) has made the process of child immunization proceed seamlessly throughout the child’s immunization schedule. The data has been synchronized and made available to pediatricians making it needless for them to keep tables, charts or hardcopy records. The data is easily available just by the click of a button. This is crucial as it enhances efficiency, systematic storage of all the immunization records, easy to access or retrieve the data, easy usage of the reports for example in guiding the public health decision making process (Namageyo-Funa et al., 2018). Utilization of the EMRs and EHRs among healthcare providers and practitioners has resulted to improved collaboration, teamwork hence makes multidisciplinary teams’ approach in healthcare possible. Therefore, there are minimal errors of commission, duplication or omission in care. Patients’ safety is guaranteed and the data from the patient can be available at every visit to the healthcare facility (De Benedictis et al., 2020).

Changes within a health care organization are pervasive nowadays as most healthcare facilities are struggling to adapt in the current volatile environment of a universal economic and political world. Some factors, such as technological innovations, professionalism, crisis, increased need for patient care, and changes in government regulations, among others, shape the process of change in most healthcare organizations (Haluza, & Jungwirth, 2015). For instance, ‘wearable techno patches’ are currently able to monitor an individual’s body temperature, heart rate, and other vital signs. In as much as this new technology has transformed the quality of healthcare, it posed a considerable challenge to most health care organizations and medical professionals, including nurses. Most patients had to be trained on how to use the gadget, and for this reason, practicing nurses had to go back and acquire adequate knowledge on the use of most technical devices that were not available during the period of their training.
Economically, most healthcare organizations had to do away with the previous electronic devices to purchase new and current machines which are very costly (McGonigle, & Mastrian, 2018). Professional nurses also incurred training costs, on the operation of the new devices to enhance the provision of healthcare services. The nursing education curriculum also had to be revised occasionally to accommodate the latest technical information. However, the social life of most patients with chronic illness was enhanced as they no longer needed close supervision form critical care nurses (DeNisco, & Barker, 2016). The changes have also resulted in the creation of new career opportunities and broaden the nursing profession in various fields such as information technology. Generally, the positive influence on healthcare organizational changes as a result of technological innovation outweighs the drawback.
