Orem’s Self-care Deficit Theory was one of the concepts I discussed last week. Orem’s concept prioritizes patient independence over self-care, with the ultimate goal of exceeding human self-care limitations (Current Nursing, 2020). When a person considers their capacity for self-care, health maintenance, and overall well-being, they are better prepared to take responsibility for their own or others’ health (Shah, et. al, 2013). Because this principle can vary greatly from patient to patient or from patient’s family to patient, its application can vary greatly depending on the group being targeted. For example, depending on age and developmental stage, this approach could be applied differently to adult and pediatric populations. This pediatric theory would almost certainly emphasize educating and assisting parents in providing care. The stage of the illness or sickness may also have an impact on how this hypothesis evolves.
In the ICU where I work, the child’s defect, arrhythmia, or illness, for example, has a significant impact on the patient’s need for self-care. In newborns with untreated congenital cardiac defects, tube feeding or oxygen therapy may be required. In this case, the parent would receive much more training on how to use and manage these modern medical devices. I could also be a young patient who was recently treated on the same unit for mechanical valve replacement. This patient may require full or partial care following surgery, but these limitations will change as the patient’s condition improves and he or she regains independence. Despite the fact that many of the teenagers in this situation have the physical and mental capacity to manage their situations,
Ultimately, because deficits can be flexible and dynamic in nature, how Orem’s Self-Care Deficit Theory is applied to a demographic focus varies greatly from scenario to situation.

I work in a pediatric CVICU and an ongoing issue has been with advancing technologies, changes in practices, and changes in charting interfaces or forms; and nursing staff willingness and acceptance in fully utilizing technology to its intended extent. There are those who have worked on this unit for a long time and don’t like the change, there are those who just miss updates, and there is inconsistency in educating staff. A great example of this can be related to Etiometry monitoring. My unit recently spent a large amount of money installing Etiometry monitors in every single patient room. When we first got it, it was explained that these monitors can collect and analyze vital patient data, help determine risk, make informed decisions on care, and reduce costs. Etiometry monitoring was supposed to collect data from patients that is often overlooked and help with anticipation of major events. However, after getting these monitors, they are often viewed as additional pieces of equipment that are often in the way. None of the nursing staff was really trained to interpret and actually understand the etiometry data. Most of the nursing staff was just taught out to turn the monitors on, and report specific numbers to providers. This has led to a disregard in terms of etiometry monitoring. Often the monitors are pushed out of the way and are not even turned on. This is just one example of how a valuable piece of technology is not being utilized; I’m sure most of us have dozens of similar examples related to equipment, charting, software, etc.
Kurt Lewin’s Change Theory could be applied to this issue, and many issues related to nursing changes in practice, and improved use of technology. Lewin’s Change theory can be summarized into three phases: unfreezing, movement, and refreezing (Current Nursing, 2020). The unfreezing phase involves overcoming resistance and creating a driving force for behavioral change. The movement phase involves the change itself or changing the thoughts or behaviors. And refreezing is making the change or new behavior habit. In terms of whether this is the best theory, I cannot say. There are so many nursing theories that can be effectively applied to many situations.

Gina integrated any form of change is a process that attracts mixed reactions. Technological change may be complicated for some nurses. However, some nurses may be flexible to incorporate change hoping that the adjustment will improve the healthcare services. Educating the staff is the right protocol towards incorporating nurses as change agents (Peukert, 2019). Technological transformation is an expensive engagement due to the gadgets’ costs. Installing Etiometry monitors in patient’s room enable the collection and analysis of vital information. The monitor fastens the patient health information collection (Lammi & Pantzar, 2019). Therefore, the monitors increase the healthcare quality and accuracy. The technological change value in healthcare services cannot be understood by nurses who have been educated about the change. Sadly, uninformed nurses may bar installing Etiometry monitors. Educating the staff allows the facility leadership to educate nurses on how to use and interfere with the data collected by the monitors. The monitors store crucial information in bulk. The patient information may be required at any time so the nurses should be flexible to retrieve the information.

Orem’s theory is an all-encompassing nursing theory that covers a wide range of topics and general concepts that apply to all nursing. Nursing is required when an adult, or the parent or guardian of a child, is unable or unwilling to provide effective self-care. Self-care deviations can occur as a result of illnesses, injuries, or diseases. There are five ways to help others: acting or doing for others, guiding, supporting, and teaching others, and providing an environment that encourages demands and future development. When nursing care is required, it is clearly stated. Her approach to health promotion and maintenance is modern, but she does not address emotional needs. This theory promotes independence, implying that all patients want to care for themselves and recover more quickly by providing their own care.
Dr. Jean Watson had firsthand experience with her theory after two life-changing events. Caring is at the heart of both nursing practice and Watson’s theory. The emphasis is on disease prevention and health promotion as a whole. Health is a high level of overall mental, physical, and social functioning, society provides values and goals, and nursing is a human science (Gonzalo et al., 2021).
Both theorists are well-known in nursing academia, having written books and spent their lives advancing and developing their theories. Both are broad theories that all nurses can employ to promote health and prevent illness. These two theories will assist the nurse leader in understanding their role, the patients’ roles, and how to promote or facilitate health balance. Humans are empowered when they can make decisions and manage their own care. Caring is essential in nursing and should be prioritized, particularly in this age of cost containment and efficiency.

An ineffective leadership trait I have observed in the perioperative area was to shorten orientation for all new employees from 1 year to 6 months at a level 1 trauma center. While on orientation, the orientees are only required to work 1 weekend shift before finishing orientation. Most do not work a weekend and get a rude awakening. On the weekends, 4 cases will be starting at 7:00 a.m… The orientee don’t think they will be on their own without a preceptor. The orientees are conditioned to call the nurses’ station for everything like a safety strap, an arm board, a doppler probe. They are not taught the location for certain items. There are no extra staff to assist because the weekend staff consists of 4 nurses, 4 surgical techs and 2 nursing assistants. The focused areas for services are Neurosurgery, Orthopedics, General, GU, and ENT. The services that are omitted are Cardiothoracic, Vascular, Ophthalmology, Gyn, and OMFS. In the trauma room, there is a CV major set, thoracotomy set, thoracotomy retractor set. What’s the point if no one was trained to use the sets? The orientees feel overwhelmed and discouraged. I try to help management brainstorm ideas on ways to make the orientees more productive. I encourage the preceptors to ask the orientees on what they would like to learn while rotating through the services to try to keep them interested and possibly motivated. Some orientees have surprised me by scrubbing in small cases like circumcisions and tonsillectomies when I ask if they would like to try. They get a sense of accomplishment and somewhat of a better understanding. I would like to see the orientees work more than 6 weekend shifts before complete orientation. I would like to pair them up with strong preceptors or more experienced staff who work other shifts. I think that exposing the orientees to different scenarios will ease the intimidation of working without a preceptor.
