Medication administration errors are flaws in medication administration that result in a difference between the drugs administered and the drugs that were supposed to be administered. Medication administration errors are common in critically ill patients’ care due to factors such as multiple medication administration, complex patient care needs, health status unpredictability, and distractions (Barakat & Franklin, 2020). Medication administration errors harm patient health by causing unintended harm, lengthening hospital stays, increasing healthcare costs, and raising operational costs in healthcare due to increased resource utilization (Devin et al., 2020). Nurses are responsible for the majority of medication administration errors due to their primary role in medication administration (Alomari et al., 2020). Medication administration errors have been increasing on the job site. Despite this, it has not looked into potential solutions, such as the use of integrated health information systems. According to the evidence, integrated health information systems are extremely effective at preventing and reducing medication errors (Jheeta & Franklin, 2017). As a result, its use on the project site is required to promote safety, quality, and efficiency in patient care, which is the project’s focus.

Propose strategies for disseminating your evidence-based practice project proposal. Consider the necessary stakeholders who need to review or approve the proposal (both internal and external) and what methods you will use to communicate.

Disseminating evidence to stakeholders and decision makers within the organization is crucial to getting the proposed solution implemented throughout the organization. The evidence must have evidence based research that supports the proposed solution and must show how the solution will improve the performance of the organization. After the completion of a research or evidence based project, the information must be disseminated amongst the staff. The purpose behind performing a research project or evidence based project is to create change and to advance patient care. The information that is distributed amongst the staff is quite important. Having a dissemination plan is so vital because there are barriers. The barriers can involve not having the education or information readily available, costs of disseminating the information to staff, and staff being resistant to the new change or information. Using a tool for a dissemination plan will help guide the researcher find all the components necessary to form a formal dissemination plan that specifically reflects the user’s needs and interests (Carpenter, Nieva, Albaghal, & Sorra, 2014). The tool provides six useful components on successfully disseminating the research findings. The components consist of defining the research, identifying target users, working with dissemination partners, communicating the research, communicating the research, evaluating the success of the dissemination process, and developing an action plan (Carpenter, Nieva, Albaghal, & Sorra, 2014). Utilizing the tool will not only provide better outcomes for the researcher but the staff members that are receiving the new information. External Stakeholders would be investors and donors. Dissemination is spreading evidence-based practice information to a variety of clinical settings to increase understanding and adoption of new or changed evidence-based practice. Proposing evidence-based practice projects in a diverse but related setting allows for the uptake of stakeholder support that would increase resources to conduct more complex studies and implement enhanced interventions. Stakeholders are very important as not only can studies require financial backing for equipment, clinical resources and personnel, but also require stakeholders that can understand the gravity a project or proposal has on improving patient related health outcomes (Cunningham-Erves, Mayo-Gamble, Vaughn, Hawk, Helms, Barajas & Joosten, 2020).

Considering my evidence-based project, internal stakeholders would primarily be health care facilities and specifically the outpatient/public nutrition department. Approval would be needed from a health care facility in order to obtain consents and access to the target population and because consent and privacy must be ethically followed approval from a health care facility may already have detailed policies regarding such proposals. Direct communication with administration and the nutrition department will be ideal to maintain good faith in stakeholder support as the project is tailored towards the nutritional field (Warren, Constantinides, Blake & Frongillo, 2021).

External stakeholders would include support from those outside of the health care facility such as local or public dieticians/nutritionists, patients, and public education programs. You can also include digital or virtual stakeholders that could support structured and guided ways to use technology for diet modification and nutritional information. While direct communication for public services and patients can be facilitated, technological stakeholder support may need technical communication for improvements and utilization (Warren, Constantinides, Blake & Frongillo, 2021).

Throughout this course, you have developed a formal, evidence-based practice proposal.

The proposal is the plan for an evidence-based practice project designed to address a problem, issue, or concern in the professional work setting. Although several types of evidence can be used to support a proposed solution, a sufficient and compelling base of support from valid research studies is required as the major component of that evidence. Proposals must be submitted in a format suitable for obtaining formal approval in the work setting. Proposals will vary in length depending upon the problem or issue addressed (3,500 and 5,000 words). The cover sheet, abstract, references pages, and appendices are not included in the word count.

Compare two organizational readiness tools. Identify the tool you selected and explain why it is most appropriate for assessing your organization.

“The ability of non-governmental organizations, government agencies, and corporations to deliver and support the availability and use of interventions for improved global public health depends on their readiness to do so” (Dearing, 2018). The COVID-19 pandemic proved that my organization as well as many healthcare organizations were not and are not ready for such public health crisis. What my organization experienced (the chaos and dysfunction) makes me question our organization readiness and whether we are even ready to implement my change proposal to increase more nurses. I say this because, when the pandemic began the organization was already cutting essential resources in the name of cost savings without properly conducting a cost benefit analysis. Our organization was short of qualified registered nurses and they refused to hire more nurses because they did not want to allocate the funds to do so even though they had enough money to hire more nurses. When it comes to presenting my idea, I foresee a backlash from upper management when it comes to allocating the funds to hire more nurses. A strategy I would propose comes from the TeamSTEPPS assessment tool. One of the steps would involve conducting a “briefing for senior leaders and key stakeholders about the TeamSTEPPS program, including its purpose and goals, what’s required from an organizational perspective, and what’s required of them to make the initiative a success; making sure they are aware of what will be needed for your organization to gain the benefits from TeamSTEPPS and allow them to make an informed decision” (Agency for Healthcare Research and Qualty, 2016). There is an abundance of current research that supports having strong nurse staffing levels and my goal is to produce the data to directly links staffing levels to patient care outcomes.

Organizational readiness can be used to learn the degree of motivation within candidates or potential organizations when implementing a process, determining particular capabilities, assisting in improving organizational capacities, and help to bring more value to their clients (Dearing, 2018). The Organizational Readiness to Change Assessment (ORCA) is an assessment tool that determines the strength and evidence provided for the intended change ro innovation, quality of the organization’s process to support change, and the organization’s capacity to facilitate and carry out change (The National Collaborating Centre for Methods and Tools, 2017). This tool helps to assess an organization’s strengths and weaknesses to support and implement evidence based practice changes. Organizational Readiness for Implementing Change (ORIC) is a readiness tool used to assess and measure the organization’s readiness for implementation and change in a healthcare setting. Not only can this tool assess the organization’s willingness to initiate a change it also observes and determines the organization’s commitment (Storkholm, 2014). For my organization I would use the ORCA too. While the ORIC can determine the organizations commitment, it focuses mainly on individuals and not the entire healthcare setting as a whole. In comparison, the ORCA can determine an organization’s strengths and weaknesses beyond the individuals initiating the change, but rather as a healthcare institution as a whole. An organization may have committed individuals but if they do not have proper funding, resources, management, supplies, equipment etc, the change will not be effective or beneficial for staff or patients.

Hello Everyone!

Welcome to NUR 590! My name is Dr. W and I will be your instructor for this course. During this course, you will be writing your evidence-based project proposal. Every week you will be writing a section of the proposal and submitting it into the assignments forum. I will provide feedback for you with each section. Please revise your sections according to my feedback and save the revisions. During week 7 you will compose the final draft of the proposal with the revised sections and the other required parts of the proposal that are described in the assignment guidelines. All of your writing must be followed by the APA 7th edition guidelines. Please purchase a hard copy of the APA manual 7th edition. I will have my copy and make references to the pages in the APA manual 7th edition in order for you to make your revisions. During the first two weeks of the course, I will post computer-assisted technique (CAT) questions for you to review APA 7th edition guidelines. The remainder of the CAT questions will be about the discussion questions.

Please become familiar with your syllabus during the first week. Please post something about yourself in the class introduction forum. There are some important due dates that are required with your initial response to the discussion questions. All initial responses to discussion question one are due on Saturday of each week by 11:59 pm Arizona time. If a response is late, 0.5 point will be deducted for each day late. The first deduction will be due to not submitting your response by the date and time. The initial response to discussion question two will be due on Monday of each week by 11:59 pm Arizona time. Moderate scores were seen in receptivity to change, compatibility, and simplicity of management.

Organizational strengths, weaknesses, and opportunities were identified from the assessment. The presence of adequate resources for implementation of the change was one of the strengths. It also showed organizational strengths of effective leadership and competent staffs that could support the change. The lack of effective management in the organization was the identified weakness since the organization lacks simplistic management systems for change. Staff shortage was identified as a threat that may make it difficult to implement change due to high workload facing the existing personnel. However, a focus on aspects such as the effective use of resources and timing of change will be used to address the above issues.

Recommended Care Process and Systems

Integrated health information technology is recommended for the organization, as it can eliminate the risk of medication errors in the organization. The integrated systems expand the provider access to patient data for accurate decision-making. As a result, adverse events in medications use will be minimized. Training the healthcare providers on the use of integrated health information systems is the recommended change in organizational processes to develop their competencies (Bouckenooghe et al., 2019).

Strategies to Facilitate Readiness of the Organization

Training of the healthcare givers will be offered to enhance competencies and readiness for change. Active stakeholder participation will be encouraged to ensure their empowerment to support the change. Openness, transparent communication will be encouraged for trust and honesty among the implementers of change (Doll et al., 2017).

Stakeholders and Team Members

Nurses, nurse managers, physicians, pharmacists, and information technology experts will implement the project. The role of nurses will be to provide insights into the system requirements needed for efficient its efficient use.
