Community health workers have the potential to improve access and quality of primary care, but they are underutilized. We described the roles and activities of community health workers in primary care by a literature analysis and synthesis to provide guidelines on their integration. Twelve functions (care coordination, health coaching, social support, health assessment, resource linking, case management, medication management, remote care, follow-up, administration, health education, and literacy support) and three prominent roles (clinical services, community resource connections, and health education and coaching) were identified after a review of 30 studies.
